I-CBT (Inference based Cognitive Behavioural Therapy) is an evidence-based therapy specifically for people experiencing OCD. According to I-CBT, obsessional doubts arise from dysfunctional reasoning characterised by a tendency to distrust our senses and over-rely on our imagination.
Inference-Based Cognitive Behavioural Therapy (I-CBT) was developed in the 1990s by Canadian researchers Dr. Kieron O’Connor and Dr. Frederick Aardema. It was created as a structured therapy model based on their research into obsessional thinking and reasoning processes in OCD.
I-CBT offers another therapy option for OCD which focuses on the beginning of the doubt rather than focussing on the the behaviour in ERP. In I-CBT, we look at how the doubt is constructed and help you to resolve inferential confusion.
Today, I-CBT is used internationally and has gained growing interest among clinicians working with OCD. It is offered by NHS Scotland, but currently, not yet within NHS England/Wales. It is in its infancy here in England/Wales.
Currently, as of 2025, the recommended treatment in the NICE Guidance (National Institute for Health and Care Excellence) for OCD, followed by the NHS, is a combination of CBT and ERP (Exposure and Response Prevention). The Guidance was published in 2005.

It explores the ‘conversations’ you have both with yourself and in your relationships – looking at where those patterns come from, and how they might be shaped by past experiences.
TA can help you notice when you’re stuck in roles like always pleasing others, feeling like you’re not good enough, being overly critical of yourself. It gives you tools to change unhelpful patterns and relate to yourself and others in a more balanced way.
It is an active, practical approach that encourages self-awareness, choice and growth.
